Abstract: The aim of this work is to modelize the occlusion of a
person with temporomandibular disorders as an evolutionary equation
and approach its solution by the construction and characterizing
of discrete variational splines. To formulate the problem, certain
boundary conditions have been considered. After showing the
existence and the uniqueness of the solution of such a problem, a
convergence result of a discrete variational evolutionary spline is
shown. A stress analysis of the occlusion of a human jaw with
temporomandibular disorders by finite elements is carried out in
FreeFem++ in order to prove the validity of the presented method.
Abstract: The aim of the study was to map scientific research on adrenal gland diseases in the Middle East countries through the Web of Science database using scientometric analysis. Data were analyzed with Excel software; and HistCite was used for mapping of the scientific texts. In this study, from a total of 268 retrieved records, 1125 authors from 328 institutions published their texts in 138 journals. Among 17 Middle East countries, Turkey ranked first with 164 documents (61.19%), Israel ranked second with 47 documents (15.53%) and Iran came in the third place with 26 documents. Most of the publications (185 documents, 69.2%) were articles. Among the universities of the Middle East, Istanbul University had the highest science production rate (9.7%). The Journal of Clinical Endocrinology & Metabolism had the highest TGCS (243 citations). In the scientific mapping, 7 clusters were formed based on TLCS (Total Local Citation Score) & TGCS (Total Global Citation Score). considering the study results, establishment of scientific connections and collaboration with other countries and use of publications on adrenal gland diseases from high ranking universities can help in the development of this field and promote the medical practice in this regard. Moreover, investigation of the formed clusters in relation to Congenital Hyperplasia and puberty related disorders can be research priorities for investigators.
Abstract: Nursing is recognized as a stressful occupation and has indicated a probable high prevalence of distress. It is a helping profession requiring a high degree of commitment and involvement. If stress is intense, continuous and repeated, it becomes a negative phenomenon or "distress," which can lead to physical illness and psychological disorders. The frequency of common psychosomatic symptoms including sleeping problems, tension headache, chronic fatigue, palpitation etc. may be an indicator of nurses’ work-related stress level. Objectives of the study were to determine psychiatric morbidity among nurses and to find its association with selected variables. The study population consisted of 1040 registered nurses working in selected medical college hospitals and government hospitals of Udupi and Mangalore districts. Descriptive survey design was used to conduct the study. Subjects were selected by using purposive sampling. Data were gathered by administering background proforma and General Health questionnaire. Severe distress was experienced by 0.9% of nurses and 5.6% had some evidence of distress. Subjects who did not have any distress were 93.5%. No significant association between psychiatric morbidity in nurses and demographic variables was observed. With regard to work variables significant association is observed between psychiatric morbidity and total years of experience (z=10.67, p=0.03) and experience in current area of work (z=9.43, p=0.02).
Abstract: Aims and Objectives: Assessment of health status and detection disorders of external respiration functions (ERF) during preventative medical examination among physicians of Armenia. Subjects and Methods: Overall, fifty-nine physicians (17 men and 42 women) were examined and spirometry was carried out. The average age of the physicians was 50 years old. The studies were conducted on the Micromedical MicroLab 3500 Spirometer. Results: 25.4% among 59 examined physicians are overweight; 22.0% of them suffer from obesity. Two physicians are currently smokers. About half of the examined physicians (50.8%) at the time of examination were diagnosed with some diseases and had different health-related problems (excluding the problems related to vision and hearing). FVC was 2.94±0.1, FEV1 – 2.64±0.1, PEF – 329.7±19.9, and FEV1%/FVC – 89.7±1.3. Pathological changes of ERF are identified in 23 (39.0%) cases. 28.8% of physicians had first degree of restrictive disorders, 3.4% – first degree of combined obstructive/ restrictive disorders, 6.8% – second degree of combined obstructive/ restrictive disorders. Only three physicians with disorders of the ERF were diagnosed with chronic bronchitis and bronchial asthma. There were no statistically significant changes in ERF depending on the severity of obesity (P> 0.05). Conclusion: The study showed the prevalence of ERF among physicians, observing mainly mild and moderate changes in ERF parameters.
Abstract: The purpose of this study are to define the nature of oral sensory issues in children with autism spectrum disorder (ASD), identify important components of the assessment and treatment of this issues specific to this population, and delineate specific therapeutic techniques designed to improve assessment and treatment within therapeutic settings. Literature review and case example is used to define the predominant nature of the oral sensory issues that are experienced by some children on the autism spectrum. Characteristics of this complex disorder that can have an impact on feeding skill and behavior are also identified. These factors are then integrated to create assessment and intervention techniques that can be used in conjunction with traditional feeding approaches to facilitate improvements in eating as well as reducing oral apraxic component in this unique population. The complex nature of ASD and its many influences on feeding skills and behavior create the need for modification to both assessment and treatment approaches. Additional research is needed to create therapeutic protocols that can be used by speech-language pathologists to effectively assess and treat feeding and oro motor apraxic difficulties that are commonly encountered in children with ASD.
Abstract: Patients with bipolar disorder are three times more likely to suffer cardiovascular disorders than the general population, which will influence their level of morbidity and rate of mortality. Bipolar disorder also affects the pulmonary system. The choice of long term-monotherapy and other combinative therapies have clinical impacts on patients. This study investigates the case of a woman who has been suffering from bipolar disorder for 16 years, and who has a history of Steven Johnson Syndrome. At present she is suffering also from cardiovascular and pulmonary disorder. An analysis of the results of this study suggests that there is a relationship between cardiovascular disorder, drug therapies, Steven Johnson Syndrome and mood stabilizer obtained from the PubMed, Cochrane, Medline, and ProQuest (publications between 2005 and 2015). Combination therapy with mood stabilizer is recommended for patients who do not have side effect histories from these drugs. The replacement drugs and combinations may be applied, especially for those with bipolar disorders, and the combination between atypical antipsychotic groups and mood stabilizers is often made. Clinicians, however, should be careful with the patients’ physical and metabolic changes, especially those who have experienced long-term therapy and who showed a history of Steven Johnson Syndrome (for which clinicians probably prescribed one type of medicine).
Abstract: Autism spectrum disorder is a complex developmental disability. It is defined by a certain set of behaviors. Persons with Autism Spectrum Disorders (ASD) frequently engage in stereotyped and repetitive motor movements. The objective of this article is to propose a method to automatically detect this unusual behavior. Our study provides a clinical tool which facilitates for doctors the diagnosis of ASD. We focus on automatic identification of five repetitive gestures among autistic children in real time: body rocking, hand flapping, fingers flapping, hand on the face and hands behind back. In this paper, we present a gesture recognition system for children with autism, which consists of three modules: model-based movement tracking, feature extraction, and gesture recognition using artificial neural network (ANN). The first one uses the Microsoft Kinect sensor, the second one chooses points of interest from the 3D skeleton to characterize the gestures, and the last one proposes a neural connectionist model to perform the supervised classification of data. The experimental results show that our system can achieve above 93.3% recognition rate.
Abstract: Retinoblastoma is a rare type of childhood genetic cancer that affects children worldwide. The diagnosis is often missed due to lack of education and difficulty in presentation of the tumor. Frequently, the tumor on the retina is noticed by photography when the red-eye flash, commonly seen in normal eyes, is not produced. Instead, a yellow or white colored patch is seen or the child has a noticeable strabismus. Early detection can be life-saving though often results in removal of the affected eye. Remaining functioning in the healthy eye when the child is young has resulted in super-vision and high or above-average intelligence. Technological advancement of cameras has helped in early detection. Brain imaging has also made possible early detection of neurological diseases and, together with the monitoring of cortisol levels and yawning frequency, promises to be the next new early diagnostic tool for the detection of neurological diseases where cortisol insufficiency is particularly salient, such as multiple sclerosis and Cushing’s disease.
Abstract: Headache is one of the most ubiquitous and frequent
neurological disorders interfering with everyday life in all countries.
India appears to be no exception. Objectives are to assess the
prevalence of headache among adult population in urban area of
Varanasi and to find out factors influencing the occurrence of
headache. A community based cross sectional study was conducted
among adult population in urban area of Varanasi district, Uttar
Pradesh, India. Total 151 eligible respondents were interviewed by
simple random sampling technique. Proportion percentage and Chisquare
test were applied for data analysis. Out of 151 respondents,
majority (58.3%) were females. In this study, 92.8% respondents
belonged to age group 18-60 years while 7.2% was either 60 year of
age or above. The overall prevalence of headache was found to be
51.1%. Highest and lowest prevalence of headache was recorded in
age groups 18-29 year & 40-49 year respectively. Headache was
62.1% in illiterate and was 40.0% among graduate & above.
Unskilled workers had more headache 73.1% than other type of
occupation. Headache was more prevalent among unemployed
(35.9%) than employed (6.4%). Females had higher family history of
headache (48.9%) as compared to males (41.3%). Study subjects
having peaceful relation with family members, relatives and
neighbors had more headache than those having no peaceful relation.
Abstract: The oral cavity can be the site for early manifestations of mucocutaneous disorders (MD) or the only site for occurrence of these disorders. It can also exhibit oral lesions with simultaneous associated skin lesions. The MD involving the oral mucosa commonly presents with signs such as ulcers, vesicles and bullae. The unique environment of the oral cavity may modify these signs of the disease, thereby making the clinical diagnosis an arduous task. In addition to the unique environment of oral cavity, the overlapping of the signs of various mucocutaneous disorders, also makes the clinical diagnosis more intricate. The aim of this review is to present the oral signs of dermatological disorders having common oral involvement and emphasize their importance in early detection of the systemic disorders. The aim is also to highlight the necessity of oral examination by a dermatologist while examining the skin lesions. Prior to the oral examination, it must be imperative for the dermatologists and the dental clinicians to have the knowledge of oral anatomy. It is also important to know the impact of various diseases on oral mucosa, and the characteristic features of various oral mucocutaneous lesions. An initial clinical oral examination is may help in the early diagnosis of the MD. Failure to identify the oral manifestations may reduce the likelihood of early treatment and lead to more serious problems. This paper reviews the oral manifestations of immune mediated dermatological disorders with common oral manifestations.
Abstract: Neurological disorders are the most debilitating of manifestations seen in patients infected with HIV. The clinical profile of neurological manifestations in HIV patients has undergone a shift in recent years with opportunistic infections being controlled with combination anti-retroviral therapy and the advent of drugs which have higher central nervous system penetrability. The aim of this paper is to study the clinical, investigation profile and various neurological disorders in HIV patients on anti‐retroviral therapy. Fifty HIV patients with neurological manifestations were studied. A complete neurological examination including neurocognitive functioning using Montreal Cognitive Assessment and HIV Dementia scale were assessed. Apart from relevant investigations, CD4 count, cerebrovascular fluid analysis, computed tomography (CT) and magnetic resonance imaging (MRI) of brain were done whenever required. Neurocognitive disorders formed the largest group with 42% suffering from HIV associated Neurocognitive Disorders. Among them, asymptomatic neurocognitive impairment was seen in 28%; mild neurocognitive disorder in 12%, and 2% had HIV‐associated dementia. Opportunistic infections of the nervous system accounted for 32%, with meningitis being the most common. Four patients had space occupying lesions of central nervous system; four tuberculomas, and one toxoplasmosis. With the advent of highly active retroviral therapy, HIV patients have longer life spans with suppression of viral load leading to decrease in opportunistic infections of the nervous system. Neurocognitive disorders are now the most common neurological dysfunction seen and thus neurocognitive assessment must be done in all patients with HIV.
Abstract: The disorders caused by the shrinking-swelling phenomenon are prevalent in arid and semi-arid in the presence of swelling clay. This soil has the characteristic of changing state under the effect of water solicitation (wetting and drying). A set of geotechnical parameters is necessary for the characterization of this soil type, such as state parameters, physical and chemical parameters and mechanical parameters. Some of these tests are very long and some are very expensive, hence the use or methods of predictions. The complexity of this phenomenon and the difficulty of its characterization have prompted researchers to use several identification parameters in the prediction of swelling potential. This document is an analytical and statistical study of geotechnical parameters affecting the potential of swelling clays. This work is performing on a database obtained from investigations swelling Algerian soil. The obtained observations have helped us to understand the soil swelling structure and its behavior.
Abstract: Pure form of L-Dopa is used to enhance muscular development, fat breakdown and suppress Parkinson disease in humans. However, the L-Dopa in mucuna seed, when present with other antinutritional factors, causes nutritional disorders in monogastric animals. Information on the utilisation of pure L-Dopa in monogastric animals is scanty. Therefore, effect of L-Dopa on growth performance and carcass characteristics in broiler chickens was investigated. Two hundred and forty one-day-old chicks were allotted to six treatments, which consisted of a positive control (PC) with standard energy (3100Kcal/Kg) and negative control (NC) with high energy (3500Kcal/Kg). The rest 4 diets were NC+0.1, NC+0.2, NC+0.3 and NC+0.4% L-Dopa, respectively. All treatments had 4 replicates in a completely randomized design. Body weight gain, final weight, feed intake, dressed weight and carcass characteristics were determined. Body weight gain and final weight of birds fed PC were 1791.0 and 1830.0g, NC+0.1% L-Dopa were 1827.7 and 1866.7g and NC+0.2% L-Dopa were 1871.9 and 1910.9g respectively, and the feed intake of PC (3231.5g), were better than other treatments. The dressed weight at 1375.0g and 1357.1g of birds fed NC+0.1% and NC+0.2% L-Dopa, respectively, were similar but better than other treatments. Also, the thigh (202.5g and 194.9g) and the breast meat (413.8g and 410.8g) of birds fed NC+0.1% and NC+0.2% L-Dopa, respectively, were similar but better than birds fed other treatments. The drum stick of birds fed NC+0.1% L-Dopa (220.5g) was observed to be better than birds on other diets. Meat to bone ratio and relative organ weights were not affected across treatments. L-Dopa extract, at levels tested, had no detrimental effect on broilers, rather better bird performance and carcass characteristics were observed especially at 0.1% and 0.2% L-Dopa inclusion rates. Therefore, 0.2% inclusion is recommended in diets of broiler chickens for improved performance and carcass characteristics.
Abstract: Lateral Geniculate Nucleus (LGN) is the relay center
in the visual pathway as it receives most of the input information
from retinal ganglion cells (RGC) and sends to visual cortex. Low
threshold calcium currents (IT) at the membrane are the unique
indicator to characterize this firing functionality of the LGN neurons
gained by the RGC input. According to the LGN functional
requirements such as functional mapping of RGC to LGN, the
morphologies of the LGN neurons were developed. During the
neurological disorders like glaucoma, the mapping between RGC and
LGN is disconnected and hence stimulating LGN electrically using
deep brain electrodes can restore the functionalities of LGN. A
computational model was developed for simulating the LGN neurons
with three predominant morphologies each representing different
functional mapping of RGC to LGN. The firings of action potentials
at LGN neuron due to IT were characterized by varying the
stimulation parameters, morphological parameters and orientation. A
wide range of stimulation parameters (stimulus amplitude, duration
and frequency) represents the various strengths of the electrical
stimulation with different morphological parameters (soma size,
dendrites size and structure). The orientation (0-1800) of LGN
neuron with respect to the stimulating electrode represents the angle
at which the extracellular deep brain stimulation towards LGN
neuron is performed. A reduced dendrite structure was used in the
model using Bush–Sejnowski algorithm to decrease the
computational time while conserving its input resistance and total
surface area. The major finding is that an input potential of 0.4 V is
required to produce the action potential in the LGN neuron which is
placed at 100 μm distance from the electrode. From this study, it can
be concluded that the neuroprostheses under design would need to
consider the capability of inducing at least 0.4V to produce action
potentials in LGN.
Abstract: Background: Maintaining the correct body balance is
essential in the prevention of falls in the elderly, which is especially
important for women because of postmenopausal osteoporosis and
the serious consequences of falls. One of the exercise methods which
is very popular among adults, and which may affect body balance in
the positive way is the Pilates method. The aim of the study was to
evaluate the effect of regular Pilates exercises on the ability to
maintain body balance in static conditions in adult healthy women.
Material and methods: The study group consisted of 20 healthy
women attending Pilates twice a week for at least 1 year. The control
group consisted of 20 healthy women physically inactive. Women in
the age range from 35 to 50 years old without pain in musculoskeletal
system or other pain were only qualified to the groups. Body balance
was assessed using MatScan VersaTek platform with Sway Analysis
Module based on Matscan Clinical 6.7 software (Tekscan Inc.,
U.S.A). The balance was evaluated under the following conditions:
standing on both feet with eyes open, standing on both feet with eyes
closed, one-leg standing (separately on the right and left foot) with
eyes open. Each test lasted 30 seconds. The following parameters
were calculated: estimated size of the ellipse of 95% confidence, the
distance covered by the Center of Gravity (COG), the size of the
maximum shift in the sagittal and frontal planes and load distribution
between the left and right foot, as well as between rear- and forefoot.
Results: It was found that there is significant difference between the
groups in favor of the study group in the size of the confidence
ellipse and maximum shifts of COG in the sagittal plane during
standing on both feet, both with the eyes open and closed (p
Abstract: Cortisol is essential to the regulation of the immune
system and pathological yawning is a symptom of multiple sclerosis
(MS). Electromyography activity (EMG) in the jaw muscles typically
rises when the muscles are moved – extended or flexed; and yawning
has been shown to be highly correlated with cortisol levels in healthy
people as shown in the Thompson Cortisol Hypothesis. It is likely
that these elevated cortisol levels are also seen in people with MS.
The possible link between EMG in the jaw muscles and rises in saliva
cortisol levels during yawning were investigated in a randomized
controlled trial of 60 volunteers aged 18-69 years who were exposed
to conditions that were designed to elicit the yawning response.
Saliva samples were collected at the start and after yawning, or at the
end of the presentation of yawning-provoking stimuli, in the absence
of a yawn, and EMG data was additionally collected during rest and
yawning phases. Hospital Anxiety and Depression Scale, Yawning
Susceptibility Scale, General Health Questionnaire, demographic,
and health details were collected and the following exclusion criteria
were adopted: chronic fatigue, diabetes, fibromyalgia, heart
condition, high blood pressure, hormone replacement therapy,
multiple sclerosis, and stroke. Significant differences were found
between the saliva cortisol samples for the yawners, t (23) = -4.263, p
= 0.000, as compared with the non-yawners between rest and poststimuli,
which was non-significant. There were also significant
differences between yawners and non-yawners for the EMG
potentials with the yawners having higher rest and post-yawning
potentials. Significant evidence was found to support the Thompson
Cortisol Hypothesis suggesting that rises in cortisol levels are
associated with the yawning response. Further research is underway
to explore the use of cortisol as a potential diagnostic tool as an assist
to the early diagnosis of symptoms related to neurological disorders.

Abstract: Current study established for EEG signal analysis in
patients with language disorder. Language disorder can be defined as
meaningful delay in the use or understanding of spoken or written
language. The disorder can include the content or meaning of
language, its form, or its use. Here we applied Z-score, power
spectrum, and coherence methods to discriminate the language
disorder data from healthy ones. Power spectrum of each channel in
alpha, beta, gamma, delta, and theta frequency bands was measured.
In addition, intra hemispheric Z-score obtained by scoring algorithm.
Obtained results showed high Z-score and power spectrum in
posterior regions. Therefore, we can conclude that peoples with
language disorder have high brain activity in frontal region of brain
in comparison with healthy peoples. Results showed that high coherence correlates with irregularities
in the ERP and is often found during complex task, whereas low
coherence is often found in pathological conditions. The results of the
Z-score analysis of the brain dynamics showed higher Z-score peak
frequency in delta, theta and beta sub bands of Language Disorder
patients. In this analysis there were activity signs in both hemispheres
and the left-dominant hemisphere was more active than the right.
Abstract: The goal of this paper is to present the diagnostic
contribution that the screening instrument, Mini-Mental State
Examination-2: Expanded Version (MMSE-2:EV), brings in
detecting the cognitive impairment or in monitoring the progress of
degenerative disorders. The diagnostic signification is underlined by
the interpretation of the MMSE-2:EV scores, resulted from the test
application to patients with mild and major neurocognitive disorders.
The cases were selected from current practice, in order to cover vast
and significant neurocognitive pathology: mild cognitive impairment,
Alzheimer’s disease, vascular dementia, mixed dementia, Parkinson’s
disease, conversion of the mild cognitive impairment into
Alzheimer’s disease. The MMSE-2:EV version was used: it was
applied one month after the initial assessment, three months after the
first reevaluation and then every six months, alternating the blue and
red forms. Correlated with age and educational level, the raw scores
were converted in T scores and then, with the mean and the standard
deviation, the z scores were calculated. The differences of raw scores
between the evaluations were analyzed from the point of view of
statistic signification, in order to establish the progression in time of
the disease. The results indicated that the psycho-diagnostic approach
for the evaluation of the cognitive impairment with MMSE-2:EV is
safe and the application interval is optimal. In clinical settings with a
large flux of patients, the application of the MMSE-2:EV is a safe
and fast psychodiagnostic solution. The clinicians can draw objective
decisions and for the patients: it does not take too much time and
energy, it does not bother them and it doesn’t force them to travel
frequently.
Abstract: Psychopathic disorders are taking an important part in
judge sentencing, especially in Canada. First, we will see how this
phenomenon can be illustrated by the high proportion of psychopath
offenders incarcerated in North American prisons. Many decisions in
Canadians courtrooms seem to point out that psychopathy is often
used as a strong argument by the judges to preserve public safety.
The fact that psychopathy is often associated with violence,
recklessness and recidivism, could explain why many judges consider
psychopathic disorders as an aggravating factor. Generally, the judge
reasoning is based on Article 753 of Canadian Criminal Code related
to dangerous offenders, which is used for individuals who show a
pattern of repetitive and persistent aggressive behaviour. Then we
will show how, with cognitive neurosciences, the psychopath’s
situation in courtrooms would probably change. Cerebral imaging
and news data provided by the neurosciences show that emotional
and volitional functions in psychopath’s brains are impaired.
Understanding these new issues could enable some judges to
recognize psychopathic disorders as a mitigating factor. Finally, two
important questions ought to be raised in this article: can exploring
psychopaths ‘brains really change the judge sentencing in Canadian
courtrooms? If yes, can judges consider psychopathy more as a
mitigating factor than an aggravating factor?
Abstract: Background: The change in foot posture can possibly
generate changes in the pelvic alignment. There is still a lack of
evidence about the effects of bilateral and unilateral flatfoot on
possible changes in pelvic alignment. The purpose of this study was
to investigate the effect of flatfoot on the sagittal and frontal planes of
pelvic postures. Materials and Methods: 56 subjects, aged 18–40
years, were assigned into three groups: 20 healthy subjects, 19
subjects with bilateral flexible second-degree flat foot, and 17
subjects with unilateral flexible second-degree flat foot. 3D
assessment of the pelvis using the formetric-II device was used to
evaluate pelvic alignment in the frontal and sagittal planes by
measuring pelvic inclination and pelvic tilt angles. Results: ANOVA
test with LSD test were used for statistical analysis. Both Unilateral
and bilateral second degree flatfoot produced significant (P
